Write the declaration yourself, in English only, in black ink on white paper. Do not write it in capital letters. The text is: 'I, _______ (Name of the candidate), hereby declare that all the information submitted by me in the application form is correct, true and valid. I will submit the supporting documents as and when required.' This form says 'submit' where other IBPS forms say 'present'. Check this against your own notice before you write it.
